Adelaide Primary Health Network (Adelaide PHN) is a not-for-profit organisation funded by the Australian Government that works to improve the health and wellbeing of people in metropolitan Adelaide by addressing local health needs and strengthening primary care. We work with primary health care professionals and communities to understand our region’s health and wellbeing needs, using comprehensive needs assessments to inform the commissioning of programs and services to deliver better health outcomes. We collaborate with general practices, allied and community health service providers, our Advisory Councils and community to identify needs and service gaps, and co-design and commission services and programs to address local needs.
