Novel approaches to support breast cancer survivorship care models

This review compares current models of survivorship care and also looks forward to describe what approaches we need to take in the future to improve the care of breast cancer survivors. It discusses six current models of care, gaps and limitations of these models along with emerging models of care; including shared care and transition to primary care. Evidence supporting new innovative technology solutions that enhance survivorship care such as web-based, telephone-based follow-up care is also sumamarised.  Overall, this article highlights how the delivery of breast cancer survivorship is evolving and that moving foward new care models need to integrate PCPs and encourage better coordination and communication between health care professionals.
