Assessment of GP-led Palliative Care in Western Australia

A Pilot Study of End-of-Life Care Registry(EOLCR)

This study aims to develop and pilot test an EOLCR configured to collect data through GPs’ practice software. Aims of this study • To test the compatibility of EOL Care Registry with Medical Director; • To examine the characteristics of palliative care patients reported by GPs; • To ascertain EOL care received by patients and GPs’ challenges in providing EOL care in community setting; • To determine how GP can be supported to early identify EOL patients, introduce palliative care, and better coordinate palliative care.
